We participate with and directly bill to all insurance companies we participate.
However, the patient is the primary and ultimate responsible for payment of services
rendered.
On the day of your first visit, evidence of current insurance coverage, your insurance card,
and a picture ID is required. Payment for any co-pay is the patient’s responsibility and due
at time of service. Co-pay varies based on your plan. We accept cash, check as payment methods.
If you do not have insurance coverage, payment in full will be required at the time service
unless arranged with our office in advance. To find out the likely charges for your scheduled
visit, please call our office during normal business hours.
Referrals and pre-authorization:
Some insurance programs require a referral from your Primary Care Doctor. Your primary physician
should provide you with the written referral order. Pre-authorization by the insurance carrier may
also be required in order for insurance coverage to be provided. Our staff will assist you in
obtaining any necessary preauthorization.
Please remember it is always the patient's responsibility to assure that proper referrals
and any required preauthorizations are obtained. If you are not sure what you need, please contact
your insurance company.
